Numb from toes down
April 30, 2009When my husband was still studying an enrolled nursing course (He just finished the course and got registered as an Enrolled Nurse already!Yey!), he brought home a copy of one of their lessons - a list of clinical notes errors that some nurses wrote down. It was very funny (the list of errors I mean) but an important lesson because it can affect the patient’s recovery.
Here’s some of the notes in the list. (Note: Please don’t try to eat or drink while reading this, it could kill you - by choking from laughing too hard..hehe..). :p

The patient refused autopsy (Duh! Even I would refuse it if I’m still living!).

Patient has left white blood cells at another hospital (Maybe he forgot it?!).

Patient has chest pain if she lies on her left side for over a year (I bet his left arm would hurt too).

On the second day, the knee was better, and on the third day it disappeared (I wonder where it went?).

The patient has been depressed since she began seeing me in 1993 (I wouldn’t want to see that doctor!).

Patient had waffles for breakfast and anorexia for lunch (Is it tasty? Anorexia?!).

The skin was moist and dry (Ano ba talaga kuya?!).

Occasional, constant, infrequent headaches. (Ano ba talaga kuya?! Part 2).

She stated that she had been constipated all her life until she got a divorce (Really? Marriage can do that to a person?!).

Skin: somewhat pale but present (Buti na lang present!).

Patient has two teenage children but no other abnormalities (Children = Abnormalities?).
These are my favorites:

Numb from her toes down (Siguro naka-upside down sya?).

Discharge status:alive but without my permission (Define discharge?).

Both breasts are equal and reactive to light and accomodation (I didn;t know it can react to light! And what’s accomodation to do with it?!).

She has no rigors or shaking chills, but her husband states that she was very hot in bed last night (That did it, I ended up laughing so hard! Good thing I’m not eating or drinking while reading this!)
Congratulations to my husband. I am so proud of you and I love you. I know you would not make the same boo-boos because you’re the best! (Syempre love your own!). :p
